Instructions for filling out Form 1583
- You may list up to 4 people TOTAL (including yourself) under the same mail box #.
- Read & follow instructions in Section 2 (VERY IMPORTANT).
- Each new applicant must have their own completed form.
- Our address (Listed above) goes in Sections 3 & 4.
- Section 4a. Pak Mail
- We will fill in your box # for you once we receive your forms and email you with that info.
- Your current address/info (NOT your new mail box address) goes in Section 7.
- DO NOT FILL IN SECTION 8a or 8b, we will fill in those for you. We simply require that you supply us with CLEAR photocopies of the ID info you use in Section 8a & 8b.
- Write "Not Applicable" in Sections: 5, 9 - 14. (PERSONAL MAIL BOX ONLY)
- After form is completed, have notary sign & stamp Section 15.
- We require original copies of all forms mailed to us, photocopies are not sufficient.
- Start up costs $10 (one time startup fee):
- Personal Mail box Cost: $12/month (for regular amounts of mail only)
- Business Mail box Cost: $24/month (required for all businesses and/or larger amounts of mail)
- Mail Forwarding cost: ALL YOU PAY IS POSTAGE (supply us with credit card info or send us check with total amount beforehand and shipping instructions).
- Please be advised - Shipping costs may vary, please contact us about pricing prior to signing up.
- Optional Services:
- - (OPTIONAL) Mail Sorting: $10/month for disposal of unwanted mail (will not be opened, instructions needed).
- - (OPTIONAL) For any packaging, mailing and any other specialty shipping costs we would need the exact details of what needs to be shipped and/or packed (i.e. Length, width, height, weight, destination, quantity, shipping service required, other instructions, etc...)
- - (OPTIONAL) For storage info contact us for prices/availability, not available for Personal Mail Box Accounts.
- All Mail Box payments are non-refundable.
